Our care management services extend to those living with developmental disabilities, including autism spectrum disorders, cerebral palsy, neurodegenerative diseases such as Parkinson’s, ALS, or Multiple Sclerosis,  as well as various mental illnesses, such as depression, anxiety, and bipolar disorder.

Our team is highly skilled in helping these individuals find housing, get proper medical care, and identify resources that will allow them to not only survive but to thrive. By connecting them with support groups, therapy, counseling, medications, job coaches and vocational trainers, we enable those living with disabilities to gain confidence, retain their dignity and maximize their quality of life. We can also manage and monitor the recommended services to ensure that the proper care is in place.
